Any health provider, a family member, or the person requiring services may make referrals to Dayton Rehabilitation Institute. Most referrals are made by a person's physician or a hospital case manager. Should you wish to refer yourself or someone else, please contact Dayton Rehabilitation Institute at (937) 424-8200.
Dayton Rehabilitation Institute personnel will rapidly respond to your referral and make arrangements for conducting an evaluation. The evaluation by the hospital is completed at no cost to you. Evaluations may be arranged at many locations, including a hospital, a doctor's office, your home, or a nursing home.
Referral Instructions
Simply call the Referral Hotline at 937-424-8215. You may leave a message if it's after normal business hours. If you are making a referral on a weekend or holiday and it needs immediate attention, call the Referral Hotline number listed above and there will be a recording directing you to the on-call Coordinator along with their cell phone number.
Insurance
Dayton Rehabilitation Institute gladly accepts the following insurances: Aetna; Anthem - Blue Corss/Blue Shield; Humana; Medical Mutual; Medicaid; Medicare; TriCare; Workers Compensation, and UHC Medicare Complete CHOICE plan.

Did You Know...?
Patients DO NOT require a 3 day qualifying hospital stay to be admitted to inpatient rehab!!
Dayton Rehabilitation Institute staff is available to perform pre-assessment screens and admit patients seven days a week!
Our staff can assess patients in the hospital, doctor's office, Dayton Rehabilitation Institute office, or in the privacy/comfort of their own home if they have great difficulty getting out!
All of our 50 rooms are private and have their own bath! We encourage family visitation/participation, child and pet visitation as well!
